Our professionals make the transition easy
The point of transfer is a particularly crucial part in the continuum of care. Individuals need to feel safe and secure. Our Care Liaisons provide emotional support to the individual, their spouse and relatives, answering questions and communicating requests to ABCM rehabilitation care centers. Prior to admission, our Care Liaisons will travel to the physician's office, hospital, or the individual's home to complete on-site assessments. Our professionals utilize our Move-In Preference Guide to gather information on day-to-day accommodations such as likes and dislikes, room preferences, meal preferences, and personal touches that make them feel at home when they arrive.
Working together to ensure continuity of care
ABCM Care Liaisons are an important part of our overall philosophy of care. These highly trained professionals offer a cooperative solution to improve communications, standardize processes, and provide ongoing assessment to ensure continuity of care between the referral source, individual and family members, and ABCM rehabilitation care centers. Our Care Liaisons follow the entire process from admission to discharge for hospitalized individuals.
ABCM Care Liaisons are available 24-hours per day, seven days per week throughout the state to meet with individuals and families, performing necessary assessments. We assist with transportation to an ABCM rehabilitation care centers utilizing our wheelchair-accessible transportation.
Uniting our resources to provide optimal care
Care Liaisons are available to help facilitate an ongoing partnership between ABCM and our referral sources. Our goal in providing this free service is to identify needs and actively seek solutions so that we can work cooperatively. Our Care Liaisons can provide expertise in regard to Medicare regulations, guidelines, and standards; keep you informed about service partnerships; and serve as a neutral resource to ensure open communication.
