Where Can I Get Additional Information about Advance Directives?

Additional information about Advance Directives can be obtained from the Iowa State Bar Association, P.O. Box 4906, Des Moines, Iowa 50306. The Bar Association will send individuals free single copies of both forms approved in Iowa. You will need to send them a stamped, self-addressed business size envelope along with your request.

You can also obtain additional information about Advance Directives from the Centers for Medicare and Medicaid Services (CMS) or from the American Geriatrics Society.
