Northgate Care Center located in Waukon, IA, is a skilled nursing facility certified in both Medicare and Medicaid. Northgate Care Center's experienced staff provides a consistent level of quality care and has embraced the practice of Person Directed Care. The Person Directed Care philosophy is used to enrich the lives of the people who live and work at Northgate Care Center by offering more choices to the staff and residents. The choices include day-to-day living decision, such as when they get up, what they eat, how and when they bathe, and what activities they attend.
The Northgate dietary department offers different freshly prepared meals daily and will gladly prepare favorite resident dishes, as well as incorporate their favorite recipes into the facility menus. An award winning activity department provides exciting and stimulating activities both in and outside our home. The transportation van is available to take residents to medical, dental, and other wellness appointments in the area.
Consistent staffing, another aspect of Person Directed Care at Northgate Care Center, ensures residents are cared for by the same caregiver at least 85% of the time. This continuity of care ensures that residents feel safe and more secure as they receive care.
Northgate's administrator, along with several other employee team members hold a Resident Advisory Board meeting with residents to maintain face-to-face communication with the residents and encourages all to express their feelings about living at Northgate. During this meeting, all departments are reviewed to ensure that the residents are being treated with dignity and respect. Residents are encouraged to share any new ideas they have to further enhance their living environment.

What's New
With a greater focus on rehab-to-home, Northgate's newly renovated rehab room gives residents a larger, more organized space to recuperate after injury. Our therapists provide excellent care six days a week to ensure that recovery is consistent. We are in the process of establishing rehab-to-home suites which will allow residents who will be with us for only a short time a relaxing and enjoyable stay.
